The following is a copy of our Priority List Information form.  Feel free to print and fill it out for each person.

PRIORITY LIST AGREEMENT

Persons wishing to assure themselves a position on the Cottages at Friends House Priority List may do so by printing, signing and delivering this agreement, accompanied by a payment of $1,000.00 of which $300.00 is a non-refundable registration fee and $700.00 is a Priority List deposit.   A position on the Priority List does not assure admission to Friends House.

1. In return for the payment of the $700.00 Priority Fee and subject to the Statement of Admissions Procedures, applicants will be considered for admission in the order of their position on the Priority List.

2. The decision to admit or not to admit an applicant is made by Friends House in the exercise of its sole discretion.   The applicant agrees to accept such decisions as binding and final in all aspects.

3. The Registration Fee of $300.00 represents recovery by Friends House of a part of its admissions cost and is not refundable.

4. An applicant's Priority Fee of $700.00 will be credited in full against the cottage Entry Fee.  The Priority Fee will be repaid in full within sixty days if an applicant is not admitted to the Community, or upon receipt by Friends House of a written request for repayment prior to signing the Lease agreement.  Friends House will not pay interest on Priority Fees under any circumstances.

5. The Schedule of Fees for the Cottages at Friends House may be changed at any time.  Written notice of such changes shall be mailed to each Priority List applicant not less than sixty days prior to the effective date of such changes.

6. The Statement of Admissions Procedures may be changed at any time.  Written notice of such changes shall be mailed to each Priority List applicant not less than sixty days prior to the effective date of such changes.

7. Should an applicant die before coming to the Community, Friends House will repay the Priority List Fee in full to the applicant's estate upon written request received within 90 days of the applicant's death.  Repayment will be made within 30 days of the receipt of the request.  If no personal representative is appointed, Friends House shall select from among the applicant's spouse, next of kin or other such responsible persons, the person to whom such payment shall be made.  Upon making selection and repayment, Friends House shall be fully discharged from further liability.

8. Except as provided in Paragraph 7 in respect to payment, an applicant's rights under this agreement are personal to him, may not be assigned, and shall not pass to his heirs or personal representatives.   If an application is make by husband and wife, both are deemed to be included in the word "applicant" as used in this agreement, and payment of a single Priority Fee and Registration Fee shall cover both.

9. It is understood that Friends House is free to expend Priority Fees for any proper corporate purpose, including costs of construction, subject to its responsibility to credit or repay the Fee in accordance with the terms of the Agreement.

10. Any notice to an applicant shall be sufficiently given if it is mailed to the address given on the Priority List Information Sheet or to an address the applicant advises, should there be a change.
