Admission Guidelines

   Welcome to Friends Skilled Nursing Facility  In order to make admission to the facility as easy as possible, we have prepared the following guidelines:

  1.     "Application for Admission" papers should be completed and returned to the facility at least twenty-four hours prior to admission.

  2.     A pre-admission bedhold charge will be effective on the day a room becomes available.

  3.     Each resident must have a physician who will follow them to Friends Skilled Nursing Facility.  The name of the physician should be made available to the nursing home staff prior to admission.  The physician must agree to visit his patient once a month for the first ninety days and no less frequently than every sixty days, thereafter.  If the physician is late in his visits, we may notify you to assist us in getting the physician to comply with the regulations.  In extreme cases it may be necessary to change your physician.  We will provide you with a list of alternative physicians if you so desire.

  4.     Our residents must have a physical examination done no more than five days prior to admission or no later than forty-eight days after admission.  If the physical is done prior to admission, papers should be picked up from the Friends Skilled Nursing Facility to be completed by the physician, and should be returned with the resident at admission.  Transfers from other facilities should be accompanied by the necessary transfer forms.

  5.     A chest x-ray must be done no more than six months prior to admission and a copy of the written report stating "no active disease or no communicable disease" must be presented before admission.  Additional tests may be required prior to admission if a resident's medical condition or history warrant such action.

  6.     Medications may not be brought into the Nursing Facility by the resident or by any member of his/her family.  A local pharmacy provides for the medication needs of our residents.

  7.     All furniture, television, radio, etc. must be sprayed for insects prior to being placed in the room; therefore, it is recommended that these be brought in prior to admission.

  8.     To facilitate the admission process, please arrive between 10 a.m. and noon on the day of admission.

  9.     All clothing to be used by the resident must be plainly marked with the resident's name.  Sew-in labels will be provided by the nursing home staff approximately three weeks after admission.   Only washable clothing should be brought to the nursing home.  Dry cleaning services are available.

  10.     If the resident is incontinent, personal laundry should be washed by the facility staff unless the family can pick it up on a daily basis.

  11.     We ask that residents refrain from bringing large sums of money, or any valuable jewelry (i.e. rings, brooches, necklaces) other than a plain wedding band.

  12.     If a living will or a durable power of health care attorney exists, we would like a copy for our resident-care files.
